The pattern repeat is worked over 13 stitches. While this is clear on the chart, it has not been fully represented in the written instructions. See this revised version for clarification:
Rnd 3: * k2, (yfwd, k3) three times, yfwd, k2, rep from * to end
Rnd 9: as Rnd 3
You may find it helps to place a stitch marker after every 13 stitches when casting on

Row 2: k1, * yfwd, sl1 pwise, yb, k1, rep from * to end

Row 4: k2, * yfwd, sl1 pwise, yb, k1, rep from * to last two sts, k2
